Um die Diagnose einer Infektion mit Histoplasma capsulatum zu optimieren, wurden 30 Milzen von mit Histoplasma capsulatum infizierten Mäusen mit der HE-Färbung, der Grocott-Färbung, einer BCG-Antikörper-Färbung und einer Fluoreszenzfärbung mit Fungiqual A sowie einer Nested-PCR untersucht. Die Ergebnisse wurden mit den Erregerkonzentrationen pro Organ verglichen, die mit Hilfe einer quantitativen Kultivierung ermittelt worden waren. Mit Hilfe der logistischen Regression wurde die Nested-PCR als die sensitivste Methode ermittelt, allerdings stellte sich heraus, dass sie nicht signifikant sensitiver ist als die Grocott-Färbung. Die Fluoreszenzfärbung und die Grocott-Färbung waren ebenfalls nicht signifikant verschieden in Bezug auf die Sensitivität. Beide Färbungen waren signifikant sensitiver als die HE-Färbung und die Immunhistologie, die sich in Bezug auf Sensitivität nicht signifikant voneinander unterschieden. Die Nested-PCR war die einzige Methode, die eine spezies-spezifische Diagnose ermöglichte.
To optimize diagnosis of histoplasmosis in tissue sections, 30 spleen specimens from mice, infected with Histoplasma capsulatum, were examined by H&E , Grocott stain, anti-bacille antibody immunostain, Fungiqual A fluorochrome stain, and a nested PCR assay. Results were compared with the tissue burden determined by quantitative culture. By applying logistic regression, the nested PCR assay was the most sensitive method, but not significantly more sensitive than the Grocott stain. The Grocott and fluorochrome stains did not differ significantly in detecting fungal elements. Both stains were significantly more sensitive than the H&E and the anti-bacille antibody immunostain, which did not differ significantly in detecting fungal elements. The PCR assay unambigously identified Histoplasma capsulatum in tissue sections.
